Terms & Conditions

Companies are required to sign a contract before they can proceed with referring any employee.

Referrals will only be accepted from Company Managers, Company Directors, Health & Safety Managers, or HR Managers for the Company Employer.

Employees must be fully informed about their referral for Occupational Health Assessment at Peak Occupational Health.

Each referral must be accompanied by the employee’s signed consent form.

All Risk Assessments remain the responsibility of the employing company and should be made available to Peak Occupational Health on request.

Opening times are Tuesdays, Wednesdays and Thursdays 9.00 a.m. – 4.00 p.m. We will only answer calls and emails between the hours of 9.30 a.m. and 4.00 p.m. We will have a phone message facility available outside working hours.

We have a strict Cancellation Policy. Signing the contract commits the referring company to a minimum of 24hrs notice to cancel, or change an appointment. The arrangements and administration of appointments, remains the responsibility of the referring company.

Full costs will be made for employee failure to attend an appointment, or cancellations made without 24hrs notice.

Travel costs are 50p per mile, and will be added to invoices for on-site visits made by Peak Occupational Health.

Payment of costs incurred obtaining medical reports from Consultant Specialists and/or GP’s, and agreed onward referrals to other Specialists, or referral to an Occupational Health Physician must be paid by the referring company and will be added to the monthly invoice.

Invoicing will be on a monthly basis and have a strict payment term of 30 days from the date of the invoice, by BACS transfer. Payments that fall outside the 30-day payment policy will incur a 15% interest charge per 15 working days of late payment.

Medical Reports and Certificates will only be submitted to the referring company after full payment of all outstanding invoice(s).

All costs are reviewed on an annual basis and will be communicated to the referring company in advance.

Peak Occupational Health reserves the right to claim expenses and terminate any contract where invoices remain unpaid after three months.
